Outcome Health, Qlik & Notitia transforming population health for Australia’s Primary Health Networks (PHNs)
Outcome Health, a Melbourne-based not-for-profit organization, has been dedicated to enhancing patient outcomes in Australia for over 20 years. With a focus on data intelligence and clinical services, the organisation is committed to improving the health system through innovative solutions tailored to community needs.
Their offerings encompass a variety of programs designed to support healthcare providers and patients alike. The suite of products and services offered by Outcome Health includes the POLAR platform, which facilitates data-driven decision-making, as well as mental health clinics located throughout Victoria. Additionally, they provide specialised clinics for diabetes and asthma within general practices and extend mental health services to Ambulance Victoria.
Despite its prevalence, healthcare data is rarely holistic. Data silos cripple the healthcare system, making a single view of a patient an often-unreachable utopia. These silos prevent healthcare organisations from leveraging their data to better improve patient outcomes. In most cases, data is siloed into different systems throughout the stages of the healthcare continuum of care.
What does this mean for the healthcare sector?
1. Disparate systems and privacy considerations make the playing field more complicated.
2. There is a disconnect between meaningful research, and the predictors that determine patient outcomes.
3. Data disconnection breeds discontent. It also means that data is not harnessed to its full potential.
4. Without a contiguous system in place, PHNs can't use data to properly understand a person’s journey through the system, or how to support better care in the future.
To address these challenges, Outcome Health forged a partnership with Notitia and Qlik to harness the capabilities of the POLAR platform. This innovative platform offers robust data transformation and analytics tools, enabling healthcare providers to explore and filter data in a meaningful and actionable way. By supporting guided analytics and quality improvement initiatives, POLAR empowers users to concentrate on critical health outcomes, thereby transforming raw data into insights that drive better decision-making and patient care.
The successful implementation of the POLAR platform has enabled Outcome Health to develop a more connected and actionable healthcare data system. By equipping healthcare professionals with clear and insightful data, they have significantly improved decision-making processes and patient care quality.
Outcome Health utilises the Qlik platform to support its POLAR reporting system, which delivers data and analytics to over 5,000 users across 1,500 general practices in Australia. The platform enhances patient health outcomes and quality improvement initiatives by transforming and presenting data effectively.
Partnering with Notitia has been instrumental for Outcome Health, as they provided expertise in managing licenses, logging support tickets, and navigating Qlik's architecture, ensuring the platform's successful implementation and ongoing support.
The POLAR dashboards feature two primary models: Data Discovery, which allows users to filter data in a structured way, likened to layering a cake, and Guided Analytics, where users follow a step-by-step process to identify specific patient cohorts for chronic disease reports. This systematic approach ensures that healthcare services are appropriately managed and that data quality is monitored.
"Having seen POLAR's benefits to the primary health sector, I look forward to pointing our powerfully cohesive data at the entire healthcare journey, to learn more about what it means to receive great treatment. With the foundation we’ve built, it might not take as long as we think"
“Working with partner Notitia has been helpful to manage licenses, log support tickets, and understand the architecture surrounding Qlik's newest functions,"
